As far as I can see these are the main types of setups:

  1. Direct Heated Mash/Lauter Tun + Boil Kettle
  2. Direct Heated HLT + Direct Heated Mash/Lauter Tun + Boil Kettle
  3. Direct Heated HLT + Unheated Mash/Lauter Tun + Boil Kettle
  4. Stovetop Infusion/Decoction Kettle + Direct Heated Mash/Lauter Tun + Boil Kettle
  5. Stovetop Infusion/Decoction Kettle + Unheated Mash/Lauter Tun + Boil Kettle
  6. Direct Heated HLT w/ Heat Exchanger Recirculation (HERMS) + Direct Heated Mash/Lauter Tun + Boil Kettle
  7. Direct Heated HLT w/ Heat Exchanger Recirculation (HERMS) + Unheated Mash/Lauter Tun + Boil Kettle
  8. Direct Heated HLT + Direct Heated Recirculation Canister (RIMS) + Direct Heated Mash/Lauter Tun + Boil Kettle
  9. Direct Heated HLT + Direct Heated Recirculation Canister (RIMS) + Unheated Mash/Lauter Tun + Boil Kettle
  10. Mash in a Bag/Basket Direct Heated Mash/Lauter/Boil Kettle

Missing anything? What about commercial setups?

I think mine would be closest to #2 (no pun intended)

Three tier system:
HLT: direct heated Keggle
Mash: direct heated 10 gal SS mega pot with recirculation via a mach pump
Boil Kettle: 12 gal SS mega pot.
