You pose a very perplexing question, one I have pondered for years. Why? Why? Why?

Did you put enough priming sugar in? Are there any yeast in suspension to facilitate carbonation? Have you waited long enough to allow for carbonation? If kegging, do you have CO2 and is your regulator working?

You are going to have to answer these questions first, before we can answer your first question.

Did you leave any head space? If force carbonating, what temp? the colder the better to force carbonate.
