On October 22, 2003 I placed 10 gals. of Czech Pils in my cooler at a controlled temperature of 45*F. Yesterday I took the two carboys out to allow them to reach room temperature of 65*F. Thats 78 days of lagering. Yesterday the surface of the wort was smooth. Today it has foam on the surface and has begun fermenting according to the bubbles coming out of the airlock every 20 to 30 seconds. I don't think I should bottle at this time. Should I leave it out and wait for it to finis or put it back in the cooler?

CO2 Outgassing
What you're seeing is the dissolved CO2 outgassing, or coming out of solution, due to the increase in temperature. Nothing to worry about. As long as you're gravities are stable, then you're fine for bottling.
